Notes on Program Questionnaires at UW

Revised January 9, 2007

Each participating UW doctoral program must submit an NRC Program Questionnaire no later than January 26, 2007.   The Graduate School will provide "pre-filled" responses to many of the questions in this lengthy questionnaire.

Definitions

Admitted students Those offered admission
Academic Year (Rev 1/7) For the purpose of the NRC study, academic year is defined as Summer Quarter through Spring Quarter, except in Section E where Summer Quarter is explicitly broken out.
Doctoral Candidacy The awarding of Ph.C. status by the Graduate School.
Doctoral Student The NRC Program Questionnaire focuses on doctoral students.  For some questions, only doctoral candidates are reported.  NRC also asks that we identify first-time doctoral students as doctoral students who entered the doctoral program even if they were initially admitted as master's students.  Please see questions C16-C18 below for a full definition of entering doctoral cohorts.
First-Time Students who were new doctoral students to the program during the report period.
Full support (Rev 12/13) The NRC defines full student financial support as follows:

"Full support can be defined as suggested by the data panel: stipend and/or salary no more than $3,000 below institutional definition of cost of living and tuition and fee remission of at least 75% of total charge to the student for tuition and fees."

For UW graduate students the minimum amounts of financial remuneration for Autumn 2005 are at least $3971 in stipend payments and at least  $2,065 in remission of Autumn 2005 tuition and fees ($4,827 for nonresidents).  All basic 50% RA/TA/SA appointments exceed these thresholds and are thereby classified as full support.

Full-time (Rev 1/9) When reporting full-time and part-time status for a particular academic quarter,  those enrolled for 10 or more credits as of the 10th day of the quarter are classified full-time and those with fewer quarterly credits are classified as part-time (e.g. questions C9a and E8).

By contrast, all doctoral students are considered full-time for questions that pertain to a range of time longer than one academic quarter (e.g. questions C2 and C16).

Mathematica Policy Research (MPR) The firm that NRC has contracted with to administer the data collection.
Modal Some questions in the student financial support section ask for "modal" support levels.  The modal amount is the most frequent amount awarded to students (i.e. the "standard" full support package).  The Graduate School does not pre-fill questions asking for the modal support levels since the mode cannot be easily or accurately calculated.  However, you may refer to the list of financial support to infer the modal level.  For further information on the statistical mode, see http://en.wikipedia.org/wiki/Mode_(statistics).

Procedures

The Graduate School has created a section in MyGradProgram for the NRC study.  This section includes areas for both the faculty lists and the Program Questionnaire.  Although NRC provides a Web-based Program Questionnaire, The Graduate School has re-hosted the questionnaire in MyGradProgram to facilitate pre-population of data.

Authorized departmental personnel can review and complete the Program Questionnaire by logging-in to MyGradProgram.  The Program Questionnaire area of MyGradProgram displays a facsimile of the Program Questionnaire with many of the responses already filled-in (designated by ).  MyGradProgram allows departmental personnel to change pre-filled responses, but not by directly changing the pre-filled numbers.  Since many of the questions are interrelated, lists are provided of the underlying data.  Departmental personnel can click from the pre-filled question to the list, edit it and save changes, returning to the modified pre-filled question.  When satisfied, departmental personnel can mark their questionnaire as finalized.

Overall Data Sources

Pre-filled data are derived from the UW Student Database and the Graduate School database of doctoral candidacy.  Specific data sources are noted under individual Program Questionnaire sections below.

Program Questionnaire Sections

Preliminary

The NRC sample Program Questionnaire asks for the name of the program, the corresponding NRC classification, and names of other programs in that classification.  These responses will be provided by the Graduate School and will not appear in the MyGradProgram version of the Program Questionnaire. 

Part A - Program Fields and Research Specialties

These responses must be provided by authorized departmental personnel using the NRC section of MyGradProgram.

Part B - Program Faculty

The NRC sample Program Questionnaire asks for a lists of core, new, and associated faculty.  This does not appear on the MyGradProgram version of the Program Questionnaire since it was obtained through the faculty list process.  Please see your faculty lists in MyGradProgram.  Questions B5 - B8 will also be completed by the Graduate School and do not appear on the MyGradProgram Program Questionnaire.

Part C - Doctoral Program: Enrollment and Degree Completion

C1   The Graduate School provides pre-populated responses for the number of doctoral degree granted.  Use the doctoral degree list to alter the data on which these questions are answered.  Only the year and quarter of admission to the doctoral program can be changed.
C2   (Rev. 1/5) The Graduate School provides pre-populated responses for the median time to doctoral degree. Use the doctoral degree list to alter the data on which these questions are answered.  Note that median time to degree is calculated as the median overall time from admission into the doctoral program until awarding the doctoral degree.  Time on-leave or otherwise not enrolled is included in the median time to degree.  Median time to degree is expressed in years, dividing the overall number of quarters by 4. 
C3   The Graduate School provides pre-populated responses for the number of students offered doctoral admission and those use accept offers and enroll as new students.  Use the offered list to alter the data on which the offered figures are calculated, and use the student cohort list to alter the data on which new student figures are calculated.
C4   This is a departmental policy.  The Graduate School has no central data regarding requirements for a prior master's degree or whether the department expects that students will normally complete a master's degree on the way to satisfying doctoral degree requirements.D
C5   The Graduate School cannot pre-populate the number of students who had a master's degree in the field prior to enrollment since "in the field" cannot be determined from central data.  This number must be equal to or smaller than the total number of new students for 2003-2006 provided in question C3.
C6 C7   The UW has a continuous enrollment policy which states that students who are no longer registered nor on-leave have lapsed from active status - see Graduate School Memorandum 9.
C8   The Graduate School pre-populates total Autumn 2005 doctoral enrollment as all doctoral students enrolled as of the 10th day of Autumn Quarter, 2005.  To alter this figure, use the Autumn 2005 enrollment list.
C9 (Rev 1/5) The Graduate School pre-populates gender, citizenship and full/part-time status for enrolled students based on the Autumn 2005 enrollment list.  Students who were enrolled, as of the 10th day Autumn 2005, for fewer than 10 credits were classified as part-time.
C10 The Graduate School pre-populates whether the program enrolls MD/PhD students from records of enrollment over the past ten years used in question C16 - C18.  The number of MD/PhD students enrolled for Autumn 2005 is derived from the Autumn 2005 enrollment list.  Students are considered to be enrolled in MD/PhD status if their first major coding in the UW Student Database is "MED C" and the second or third major is a major in the program.
C11 The median time to degree for MD/PhD students is calculated as the overall time from the first instance of major coding "MED C" to awarding of the degree.  Time on-leave or otherwise not enrolled is included in the median time to degree.
C12 If your program has a definition that extends beyond the Graduate School's definition of equal or greater than 10 credits, indicate here.  Otherwise specify 10 or more credits.
C13 Answer yes if your program has formally established requirements for admission to candidacy.
C14 Please mark all that apply.  You must mark at least one.
C15 NRC is asking this question to understand limitations, if any, in the response to questions C16-C18 which ask about completion rates of doctoral students.  These data are less useful if no distinction is made between master's and doctoral students.  The Graduate School's method of pre-populating this data identifies students who get a master's degree and continue to the Ph.D. as well as those admitted into the Ph.D. program

These questions are pre-populated by the Graduate School.  Departmental personnel may add/remove or change attributes of student cohorts by navigating to the cohort list in MyGradProgram.  Doctoral student cohorts were identified as:

A) admitted into an NRC eligible program as a doctoral student (normally degree level 3 or 4).
B) admitted to a terminal master's program, earned a master's and immediately began the pursuit of an nrc study eligible PhD in the same major (the student must have at least one quarter coded as a degree level 3 or 4 in the program).
C) enrolled at least one quarter as an MD/PhD (coded as a 'MED C' major) AND were also enrolled in an NRC eligible PhD program.

Students that fall into category B above have their starting (entering cohort) year/quarter as one academic quarter following the receipt of the master's. Students in category C above have a starting (entering cohort) year/quarter as the first quarter enrolled in the NRC eligible PhD program.
C19 The Graduate School pre-populates the names of students in your program who were awarded doctoral degrees from Summer 2003 through Spring 2006. 

Part D - Doctoral Program: Characteristics

D1 Since all programs in the NRC study require the GRE, the Graduate School has pre-populated this response as "Yes".
D2 Since there is no exemption from the GRE requirement, this question should remain blank.
D3 Check "Yes" since all UW doctoral programs currently require GRE scores
D4 The Graduate School pre-populates the number of GRE takers and median GRE scores from scores loaded into the UW Student Database from Educational Testing Service for new doctoral students in the years specified.

The definition of Teaching Assistants here includes Pre-doctoral Teaching Associates and Pre-doctoral Lecturers.  Terms here refers to academic quarters.
D7 The Graduate School does not have central data on specific responsibilities and roles of teaching assistants and cannot pre-populate this question.  However, a list is provided in MyGradProgram for guidance.
D8 As a guide to filling out the question on support resources, please consider that there are institutional resources for the following:
  • International student orientation
  • Language screening/support prior to teaching
  • Instruction in writing (outside of program requirements)
  • Instruction in statistics (outside of program requirements)
  • Prizes/awards to doctoral students for teaching and/or research
  • Assistance/training in proposal preparation
  • Formal training in academic integrity/ethics
  • Active graduate student association
  • Posted academic grievance procedure
  • Dispute resolution procedure
  • Regular graduate program directors/coordinators meetings
  • Organized training to help students improve teaching skills
  • Travel support to attend professional meetings
D9 Awards conferred to faculty may or may not include remuneration, and may be recognized only within the program.
D10 The Graduate School collects employment plans for doctoral students in the Graduate School Exit Questionnaire and aggregated results are available in MyGradProgram.  Additionally, the Office of Educational Assessment (OEA) conducts a graduation survey approximately eleven months following graduation.  Results for the OEA survey are available at http://www.washington.edu/oea/pdfs/reports/gradsurv03.pdf .

Part E - Doctoral Program: Financial Support for Full-Time Students

E1 The Graduate School pre-populates 2005-06 tuition and fees based on full-time enrollment in tuition tier 1 programs for Autumn, Winter and Spring Quarters.  The amounts include the quarterly Technology Fee ($40) and Services & Activities Fee ($94) and IMA Bond Fee ($35).  This amount is $8,257 for residents and $19,307 for non-residents.  Source: http://www.washington.edu/students/sfs/sao/tuition/ttn05-06.xls.

Two health insurance packages are available for graduate students; Graduate Appointee Insurance Program (GAIP) for graduate appointees (http://www.washington.edu/admin/hr/benefits/gaip/premiums.html) and the Accident and Sickness Insurance Plan (http://depts.washington.edu/ovpsl/insurance.html) available to all students.  The Premium for the former ($371 per quarter or $1113 for the academic year 2005-06, student only) is used as the pre-populated value.

E2 The Graduate School does not pre-populate the modal amount of total financial support, but a list is provided in MyGradProgram, sorted by total dollar amount, for guidance.  Since the modal amount is simply the most frequent package expressed in gross dollars, the list provides what is known of financial support that is processed through the UW Payroll/Personnel system (TA/RA/SA/Major Fellowship/Trainee).  Student support provided by funds not processed through the UW should be included. 
E3 A MyGradProgram list is provided for guidance including funding levels of direct gross stipend payments for full-time (10 or more quarter credits) first-time (students with NCR codes 1,3, or 4).  If your modal support includes a waiver of tuition and fees, please include the amount from E1.  If your modal support includes health insurance premium coverage (under the GAIP plan), please use $xxxx for the academic year.
E4 (Rev. 12/19) A MyGradProgram list is provided for guidance (see E3).  Full support for those supported Summer 2006 was defined as receiving a stipend of at least $1406 which corresponds to the basic TA/RA stipend for one month.  Those receiving full support must also receive remission of tuition and fees of at least $579 for residents and $1369 for non-residents.  These tuition amounts are based 75% of the amount of the prevailing tuition rates for 2 credits.  The period of support is 6/16/2006 through 9/15/2006.
E5 The Graduate School's interpretation of NRC full support is listed under definitions above.  Students receiving an equivalent level of support through combined supplemental sources are also considered fully supported.  See Graduate Student Service Appointment Salary Schedule at http://www.grad.washington.edu/fellow/7-05-regular-GSSA-salary-schedule.pdf.

E8 (Rev. 12/13) The Graduate School pre-populates the Fall 2005 support by type, level and years in program.  Numbers in the grid correspond to headcounts for full-time doctoral students.  See C16-C18 for methods used to identify doctoral students and for the list of students with their level and type of support.  Financial support for the list is derived from the UW HEPPS Payroll system for graduate student teaching, research, and staff assistantships, and for fellowships and traineeships.  Payments for tuition, scholarships and other disbursements through the Student Accounts database are also included.  Pre-populated amounts are broken out by stipend, tuition, and GAIP health insurance benefit, and other.  The health insurance amount will be $371 per student on Schedule 1 appointments or holding a fellowship appointment with job classification codes 0863 or 0864.  Additional dependent coverage is not considered.   Other includes unspecified payments though Student Accounts which may include scholarships.

(Rev. 12/18) Support is indicated for your program's students regardless of the source of support.  For example, a student in your program who held a TA in another program will have support reported in your program.

Type of support indicated on the list is derived from the combination of all funds received (as described above) from 9/16/05 - 12/15/05.

Externally funded fellowships and traineeships are those funded by a non-UW and non-State of Washington source.  Institutional fellowships are those funded by an internal University of Washington, local government or State of Washington source.

Those with less than qualifying amounts are classified as "Funded with less than full support" and those without appointments in HEPPS for Autumn 2005 (any active payroll distribution of at least five out of six pay periods between 9/16/05 - 12/15/05) and no associated payments through Student Accounts are classified as "Unfunded".  You may edit the student list to change the support level and type if the student receives funding from other sources not pre-populated in the list.  Since the pre-populated data includes only external funding that is processed through the UW HEPPS or Student Accounts systems, you may want to change student entries when they receive external support that is not processed through these UW sources.

Do not include personal, spousal, or family support, wages from work unrelated to the program, or loans.

E9 The Graduate School does not pre-populate this question.

Part F - Postdoctoral Scholars

The Graduate School is not pre-filling this section, but will provide a list of postdoctoral scholars as a guide.  The list will be compiled from the UW Payroll Personnel system by the Graduate School.  Graduate School is not attempting to pre-populate postdoctoral data since there is no effective way to relate the postdoctoral scholar's organizational unit to graduate programs, and postdoctoral scholars may be paid from non-UW sources.  The intent is to identify postdoctoral scholars who work with core or new faculty.

F1 Postdoctoral scholars as defined by NRC/AAU are typically paid using one of the following job classification codes:

0148 Research Associate
0150 Research Associate Trainee
0442 Senior Fellow Trainee
0445 Senior Fellow

Please list only postdoctoral scholars who work with core or new faculty in your program.

F2 Please use the provided list in MyGradProgram to assist in completing this question.

F4 Although portable fellowships are awarded directly to the postdoctoral scholar, the funds may be processed through the UW payroll system.
